Approval packets

Once your grant has been approved, you will receive an approval letter and packet with useful materials and a guide to get started. You do not have to wait for your approval letter or packet to use your grant—you can use approved funding immediately.

If you applied online:
Your digital welcome packet materials, as well as your patient and grant information, can be found in your portal account. To pay for prescriptions at the pharmacy, find your TotalAssist Pharmacy Card in your portal account. You can access your information and submit claims 24/7 in your portal account.

If you applied by phone:
The support specialist will make sure you have the information you need to pay for prescriptions at the pharmacy right away. You will receive a welcome packet and approval letter by mail in a few days. For 24/7 online access to your grant, create a TotalAssist portal account. You can track your balance, submit claims, check claim payments, and more.

What is in your approval packet

Approval letter. Your approval letter will include:

  • Patient ID number
  • Approval date
  • Eligibility period dates
    • You can submit claims for expenses incurred between your lookback date (6 months prior to approval date) to the grant expiration date (12 months from approval date)
    • Date of the last day you can submit claims for your grant
  • Grant amount, including guaranteed and maximum amounts
  • Date you are eligible to reapply

Your packet will also include:

  • Pharmacy card: Take this card with you to the pharmacy to pay for prescription medications covered by the program. Funds that do not include a pharmacy benefit will not include this in their welcome packet. Learn more about pharmacy cards
  • Claim guide: Contains instructions on how to submit a claim for payment.
  • Claims form: Use this form each time you submit a claim to the program.
  • Patient agreement, disclosures & attestations: This is your copy of the agreements you (or your authorized representative) accepted during the application process.
  • Electronic Fund Transfer (EFT) form: As part of Patient Advocate Foundation’s TotalAssist program, you can have reimbursement sent directly into your checking account. It is a quick and easy way to get paid for your claims. Anyone using EFT must have an open checking account. Learn more about signing up for EFT

For transportation grants, your packet includes:

  • Approval letter: Provides details of your grant and other important information.
  • Patient agreement, disclosures & attestations: This is your copy of the agreements you (or your authorized representative) accepted during the application process.
  • Instructions to activate your pre-paid VISA card: Provides instructions on how to activate your pre-paid VISA card online. Learn more about your travel card

Can I request a copy of my approval packet?

If you are approved for a TotalAssist grant, you will receive an approval packet by mail or email, depending on your preferred contact method. You can also access your approval packet materials in your portal account. If you need another copy and have a portal account, you can download it directly from the portal. You may also request a replacement copy by contacting us.
