About health equity funds

What are health equity funds?
Where you live can impact your access to healthcare support, especially in areas with higher rates of disease and less resources. Our Health Equity Funds offer financial help to eligible patients living in areas identified as having increased social and financial needs, as well as higher rates of some diseases and chronic conditions.
Eligibility
Eligibility for a Health Equity Fund is determined first by the zip code of your home address. Additional eligibility requirements are the same as a general disease fund.

How we identify Health Equity Funds
We recognize that patients living in some areas have unequal access to resources, care, and safety net services. Patient Advocate Foundation is committed to providing help to patient communities in need. We determine where to provide more financial assistance through Health Equity Funds using real U.S. population data. We combine data from the Centers for Disease Control and Prevention’s (CDC) Social Vulnerability Index (SVI) and Disease Incidence data. Currently, our Health Equity Funds support over 226 counties across 36 states with the highest rates of social vulnerability and high rates of multiple chronic diseases.
