Eligibility for TotalAssist financial assistance is based on verification of a qualified diagnosis and demonstrated financial need. If we are unable to verify diagnosis and/or financial need within 30 days of your approval date, your grant will be forfeit and closed.

If you still need assistance and can provide proof of your household income, please call us at (866) 512-3861 to complete a new application or apply online at TotalAssist.org.
You may reapply for funding if:

- Funding is available
- You meet all program requirements
