How we verify that your income meets eligibility guidelines
At the time of application, TotalAssist uses a third-party income verification service to confirm that the income, household size, and state/zip code information you provided in your application is within the fund’s eligibility guidelines.
Typically, no further action is needed by patients. In some cases, we will need to verify the income reported on a patient’s application, and we will request proof of household income that must be completed within 30 days.
If you received a letter requesting proof of income
In some cases, we will need to verify the income reported on a patient’s application. We will send you a letter requesting that you submit proof of household income. Depending on the preference you indicated, you will receive a letter by mail or by email. The letter will also be posted to your portal account. The proof of income documentation must be submitted to TotalAssist within 30 days of application date for continued eligibility. Patients cannot submit claims for payment during this income verification period.
If applicants do not comply with the request for income documentation within 30 days, the application will be closed. The patient will need to reapply if funding is available
Upon TotalAssist’s review of submitted income documentation, approved patients who have a household income higher than program guidelines will not qualify for support.
What to send as acceptable proof of income
- A copy of your signed federal tax returns from the most recent tax year (pages 1 and 2 only) are the preferred. Your tax return provides proof of household income.
If you are unable to send your tax return, please see below for acceptable documents based on what applies to your household income. While not required, we ask that you send as many of the listed documents as you have for the scenario(s) that apply to your household.

| Is employed | Your W2. Three (3) recent consecutive paystubs. A letter from your employer stating your salary. |
| Receives Social Security Income (SSI) | Your SSI statement. Your bank statement that shows the monthly SSI deposit that you get each month. |
| Is retired | Your pension or retirement benefits statement. Your bank statement showing the monthly retirement deposit that you get each month. |
| Is disabled | A statement of your short-term disability (STD) and/or long-term disability (LTD) benefits. |
| Is divorced or a single parent | A statement of alimony and/or child support you receive. |
| Has been injured on the job and can’t work | A statement of workers’ compensation. |
| Earns interest, or owns stocks, bonds, or mutual funds | A statement of interest income and/or dividends earned on your account. |
