Health Equity Funds offer help to eligible patients living in areas identified as having increased social and financial needs, as well as higher rates of chronic conditions. Eligibility is determined in part by home address zip code.
TotalAssist also offers a type 2 diabetes fund. Please note that you cannot receive more than one grant for the same condition.
Eligibility criteria
To be eligible for this fund, you must:
- Have a confirmed diagnosis and are in treatment for type 2 diabetes, or planning to begin treatment in the next 60 days, or have been in treatment in the past 6 months. This includes active surveillance of a condition
- Have government-insured coverage that covers your qualifying expenses
- Have an income at or less than 500% of the Federal Poverty Level, adjusted for your regional Cost of Living Index (COLI). What is this?
- Be a legal resident and receive treatment in the U.S. or a U.S. territory
- Be prescribed a medication or product listed in the approved medications, listed below
- Have a home address in a zip code served by a Health Equity Fund, which supports eligible patients living in designated social vulnerability counties

About the disease
Type 2 diabetes (T2DM), also known as Type 2 diabetes mellitus or adult-onset diabetes, is a chronic disease that affects the endocrine system. Patients with Type 2 diabetes do not respond to insulin or do not produce enough insulin, which results in high blood glucose levels.
