This TotalAssist fund helps you cover copays, coinsurance, and deductibles related to charges for unmet social needs screenings or services conducted by your healthcare provider, including administration of Social Determinants of Health (SDOH) assessments, Community Health Initiating (CHI) services, and Principal Illness Navigation (PIN) services covered by your insurance. This fund supports patients diagnosed with having unmet social needs, specifically challenges related to access to food, transportation, housing, and other economic circumstances that impact their health, ability to access necessary care, and ability to stay on treatment.
Eligibility criteria
To be eligible for this fund, you must:
- Have been diagnosed by your provider with having an unmet social need impacting your health or access to needed care
- Have health insurance that covers your qualifying expenses
- Have an income at or less than 500% of the Federal Poverty Level, adjusted for your regional Cost of Living Index (COLI). What is this?
- Be a legal resident and receive treatment in the U.S. or a U.S. territory
